- A professor, mentor, employer, former employer, or the like should complete this form. This form cannot be completed by a relative.
- Homeschool Applicants: Students under 21 years of age will need to submit SAT or ACT scores. Multnomah’s standardized test identification numbers are:
- SAT Code: 4496
- ACT Code: 3476
- Request your Official Transcript(s)
- Undergraduate
- This should include at least the junior year (or GED Score Report) from your high school. It should be a sealed document sent directly from your high school to Multnomah University. If you have not yet graduated, please also submit a final transcript upon graduation.

- Official Transcript(s) from each college or university you have attended, or are currently attending, or from which you will receive credit.
- Transfer applicants who have earned fewer than 24 semester hours (or 36 quarter credits) of college-level credit must also submit high school transcripts or GED results.
- Homeschooled Students
- Multnomah provides a free transcript template as a courtesy to our homeschooled applicants.

- Designate a Mentor
- Online M.A. in Christian Leadership and Master of Divinity only: As a part of Multnomah Biblical Seminary’s online programs, students are required to meet regularly with a mentor (Pastor, Elder, Deacon, or other Church Leader) in their local community. This form is to be completed by the person agreeing to serve in that capacity.
